The Know Diabetes service is working with over 350 GP practices in Hammersmith and Fulham, Kensington and Chelsea, Westminster, Ealing, Hounslow, Brent, Hillingdon and Harrow to invite people with diabetes and those at risk of type 2 to sign up for the Know Diabetes service.

*If you are registered with a GP practice outside of the eight North West London Boroughs (mentioned above), we are currently unable to provide you with access.

How can I get updates on the Corona virus and diabetes?
When you register your interest for Know Diabetes we will email you to ask what your information preferences are.
If you select that you want information on the coronavirus we will keep you up to date with the best advice.

When will my practice be invited to create their Know Diabetes account?
The first group of patients to be invited will be those who have an email address registered to their GP record. We recommend that you register your interest here today so we can alert you when this full service is being offered to your practice, as part of the roll-out plan.
How do I Login?
Via the login page at the top of the page. If you have not been invited to create your account by the Know Diabetes service you will not be able to log in yet. Your invitation to create an account will be emailed to you from drtonywillis@knowdiabetes.org.uk. We recommend that you register your interest above so we can alert you when this service is being offered to your practice.
